1. Under this special-price promotion, LASIK/SiLK™ procedures may only be performed on August 24 or 26, 2026.
    
2. To register for the promotion, participants are required to pay a commitment fee of Rp 12 million through one of the following payment methods:
   • Online payment through www.klinikmatanusantara.com
   • Payment at KMN EyeCare cashier
   • Bank transfer to BCA:
     - Account number: 483-054-8888 (for LASIK at KMN EyeCare Kebon Jeruk) or
     - Account number: 483-074-9999 (for SiLK™ at KMN EyeCare Kemayoran)
     - Account holder: PT Klinik Mata Nusantara
     - Proof of payment must be sent via WhatsApp to: 08118505669 or by email to Marketing@KMNEyeCare.com
      
3. Payment can be combined with a 3-month 0% installment program with a BCA credit card or 12-month 0% installment program with a credit card from any of the following partner banks: Mandiri, BCA, Mega, BNI, Maybank, Danamon, DBS, and HSBC.
    
4. If a participant is determined by the doctor to be medically unsuitable for LASIK/SiLK™, the commitment fee will be fully refunded 100%.
    
5. The commitment fee is non-refundable if the participant cancels the procedure for personal reasons.
    
6. LASIK procedures are available only at KMN EyeCare Kebon Jeruk, and SiLK™ procedures are available only at KMN EyeCare Kemayoran.
    
7. LASIK/SiLK procedures can be performed by all Doctors who do LASIK/SiLK™ at KMN EyeCare.
    
8. Initial examinations, pre-LASIK/SiLK diagnostic examinations, and post-operative examinations may be conducted at any KMN EyeCare branch.
    
9. Participants must meet the general eligibility requirements for LASIK/SiLK™, including:
   • At least 18 years of age
   • Must discontinue soft contact lenses for at least 2 weeks, or hard contact lenses for at least 3 weeks, before the initial screening or pre-LASIK/SiLK examination
   • Female participants must not be pregnant or breastfeeding
   • Must not have thyroid disease
   • Must not have a history of autoimmune disease
      
10. Participants must follow KMN EyeCare on social media accounts on Facebook, Instagram, TikTok or X (you may choose one).
     
11. Participants are required to post a photo or share their experience of undergoing LASIK/SiLK at KMN EyeCare on their personal public (non-private) social media account for a minimum of one (1) week. The post must include the hashtag #LASIKKMN or #SiLKKMN and tag the official KMN EyeCare social media account.
